Community Health Worker (Part-Time) - Orthopedic Surgery
Scheduled Hours20Position SummaryThe Department of Orthopedic Surgery is looking for an engaged candidate that will build partnerships with patients and the community. Working toward addressing social determinants of health to promote adherence to prescribed treatment plans. The ideal candidate will provide assistance within Complex Nerve/Hand division of the Department of Orthopedic Surgery, providing guidance to patients and their families as they navigate through community based support services. Meeting with patients face to face or via telehealth based on referral requests from Providers, Advanced Practice Providers, or Clinical Team Members.
