Primary Function The Coordinator manages the caseload of FXW students on the Student Success Team (SST), specifically the implementation of the students IEP and ILP plans in inclusive, self-contained and pull-out settings. The Resource Coordinator is a part of the Education Team and works closely with Principals and Admissions Team. For students/families: Effectively communicate to parents and guardians the SST process including referrals, evaluations, interventions, and CPS IEP/504 Engage parents and families in their students learning and acting as an ambassador for the school in the community. Connect with students outside providers, pediatricians, and therapists to support students needs in classroom. Coordinate student progress meetings with FXW SST, outside providers and parents/guardians: beginning with internal communication and following with next steps email to all attendees Maintain student files: including annual release of confidential information forms Coordinate services for students with a CPS IEP: scheduling, develop service plans and progress monitor and maintain information to meet local, state and federal audits. Track evaluations, re-evaluations, CPS eligible IEP/504 plans Facilitate ILP meetings using a strength-based approach For teaching colleagues: Coordinate with students SST to ensure all documents are completed: intervention plan, CPS applications, evaluations Coordinate internal SST meetings (Thursdays) reinforcing the FXW referral process Ensure Individualized Learning Plan (ILP) and Intervention Plans (IP) are developmentally appropriate, curriculum/standards -based, strength based, and relevant to individual students. Initiate and support the planning of interventions with the FXW SST Identify and develop appropriate curriculum and school-based assessments to support the academic growth of students with ILPs. Coordinate Tests for Aimsweb Plus, WrAP and MAP Job Description Provide leadership with the development of program design, special materials, curriculum, and methods. As a part of the Education Team: Plan and conduct SST meetings for Staff Days throughout the year. Assist Admissions Team with screening of applicant candidates. Coach assigned personnel in the development of and implementation of intervention plans. Key Relationships The Coordinator reports to the Director of Education. Other key relationships he/she is responsible in cultivating are with principals, teachers, clinical psychologist, and Admissions Team. The Coordinator supervises members of the SST as designated by Director of Education.
Qualifications and Requirements A candidate should hold: • A Bachelors or Masters Degree in Special Education is preferred and/or Masters in Educational Leadership. • A valid, issued State of Illinois teaching certificate Type 03, 04, 09, 75 • Three years of successful teaching experience with students A candidate will need upon hiring: • Comply with Archdiocese of Chicago required Protecting God’s Children Requirements (Background Screening, Fingerprinting, VIRTUS Training, CANTS, Code of Conduct, Mandated Reporter Training) A candidate must demonstrate: • The ability to establish collaborative relationships with teaching staff through a supportive supervision and evaluation model. • Familiarity with ISACS criteria and standards of excellence
