The Ag Worker Health Services Coordinator will act as a liaison between Community Medical Centers and agriculture worker communities and organizations serving Ag workers across Community Medical Center’s service area. Responsibilities include the development of collaborative partnerships, coordination of outreach activities, social media and communication tools, and working collaboratively with CMC Clinical team to ensure CMC clinical programs effectively meet the needs of agricultural workers.

Facilitate projects and initiatives relevant to Ag Worker Health Services
Develop, and maintain collaborative relationships with service area organizations serving the Ag Worker Community such as El Concilio, Catholic Charities )etc.
Develop and support an internal CMC Ag Worker Advisory Committee composed of both clinical, program staff and CMC Board Member representation. Advisory Committee will identify and address health equity priorities in the Ag Worker population.
Work collaboratively with Health Education, Outreach and Enrollment staff to ensure services are culturally and linguistically appropriate for Ag Worker populations.
Ensure CMC programs are available and known to Ag Workers
Pilots and evaluates variety of Ag worker engagement strategies
Identifies and seeks to reduce barriers to care and programs
Coordinates patient satisfaction focus groups
Educates patients on patient portal
Participates in organizational and external meetings as required
Travels as needed to different sites as assigned
Performs other tasks as assigned
Knowledge, Skills and Abilities
Knowledge of business office procedures
Knowledge of Excel (Intermediate; proficient experience preferred)
Skill in operating computer, photocopy and facsimile (FAX) machine
Ability to establish and maintain effective working relationships with patients, employees, and the public

TYPICAL PHYSICAL DEMANDS:
Work may require sitting for long periods of time; also stooping, bending and stretching for files and supplies. Occasionally lifting files or paper weighing up to 35 pounds. Requires manual dexterity sufficient to operate a keyboard, type at 35 wpm, operate a computer, calculator, telephone, copier, and such other office equipment as necessary. Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts. It is necessary to view and type on computer screens for long periods and to work in an environment which can be very stressful.
TYPICAL WORKING CONDITIONS:
Work performed in reception, medical records, and cashier areas. Involves frequent contact with patients. Work may be stressful at times. Interaction with others is constant and interruptive. Contact involves dealing with sick people. Frequent exposure to communicable diseases, toxic substances and other conditions common to a clinic environment.

High School Diploma/GED with 4 years’ experience; AA degree with 3 years’ experience; or Bachelor’s Degree with 1 year experience.
Experience should be in a patient-facing role with emphasis on education and coaching
Social media marketing experience preferred
Community organizing, patient navigation or community outreach experience
Proficient in using Microsoft Office (including Word, Excel, and PowerPoint)
Excellent interpersonal and networking skills
Excellent organizational skills to manage a variety of tasks (multi-task)
Ability to lead and work collaboratively in team settings
Proficiency in effective and professional communication, both verbally and in written format
Valid California Driver's License, proof of insurance and personal transportation
Ability to effectively manage projects, multi-task and prioritize
Ability to interpret and develop processes, including asking questions to clarify and fully understand procedures
Bilingual Spanish, with ability to read, write and translate materials.

About Community Medical Centers
Community Medical Centers, Inc. (CMC) began in the 1960's as a volunteer effort of the San Joaquin Medical Society, the San Joaquin Local Health District and the Community Action Council. Local physicians, nurses, dentists and community activists who recognized the lack of health and social services programs formed service teams to address the needs of migrant farm workers and their families. The providers went out to the fields and worked from their cars to deliver medical care, to supply food and clothing and to link families with available services. In 1967, the San Joaquin Medical Society received state and federal funding to support the development of two small facilities, as well as mobile clinics to provide services throughout the county. From these early beginnings, CMC has grown to provide comprehensive primary and preventive care at eleven clinical sites in the counties of San Joaquin, Solano, and Yolo.
