Recruitment Incentives are not Authorized. This position is eligible for the Education Dept Reduction Program (EDRP), a student loan reimbursement program. To participate in the program, you must meet specific individual eligibility requirements in accordance with VHA Policy 1021 and submit your application within four months of appointment. The final award amount (up to $200,000) and eligibility period (one to five years) is determined by the VHA Education Loan Repayment Services office. Functions a Physician for the Patient Aligned Care Team (PACT), Ambulatory Care Services, at the H. John Heinz III VA Campus, Pittsburg VA Health Care System. May perform additional duties (temporary or permanent) as a float provider (face-to-face or via telehealth) as those duties are assigned. Responsible for providing medical care in an outpatient setting following PACT principles. Will routinely provide medical care using Face-to-Face(F2F), VVC, and/or Telephone appointments. Assures Veterans receive compassionate, efficient and safe patient care in their respective care line. Assures clinical care provided meets all timelines and required documentation as required by VA. Providers must review alerts daily and have no more than 200 alerts at any given time. They must encounter their notes within 72 hours. Encounter errors must be completed within 7 days. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran. This includes daily and weekly huddles with your PACT. Attend either by skype or in person all Primary Care statewide meetings and whenever possible, participates in the clinical and teaching activities at MTVAHCS Ambulatory Care statewide meetings. Comply with all directives and policies related to space management to achieve optimal resource utilization (space, professional and administrative) to maximize clinic utilization within the PACT model. Improves quality of care by achieving: Excellence in meeting national and VISN 4 patient safety goals. Participates in PACT huddles and manage your PACT patients with diabetes, hypertension, heart disease and mental health disorders (HEDIS) with the goal to reduce your PACT metrics and obtain excellence of HEDIS metrics, Excellence in ORYX (Joint Commission) measures. Patient Care: Medical Knowledge (evidence-based medicine), Practice-Based Learning and Improvement, Interpersonal and Communication Skills, Systems-Based Practice. Professionalism: Practice the principles of the Pittsburg VA HCS's Civility in the Workplace policy. Promote civility, through your actions, among employees and the Veterans we serve and actively participate in an environment where people from all walks of life embrace and practice respect, courtesy and consideration. Through your actions, maintain a workplace that is effective and productive. Support civil interactions with peers, colleagues & staff that create the best possible environment for providing excellent care to Veterans. Practice courteous and positive communication without belittling, being rude, and showing hostile behavior. Complaints from staffing or clinic personal about provider professionalism will be taken seriously and steps to ensure workplace safety will be taken. Thorough knowledge of clinical and administrative procedures as they apply to inpatient and outpatient care. Knowledge of the internal organization and content of patient's medical records and the computerized patient record system (CPRS) in order to search records and extract data relevant to completeness, accuracy, and consistence with medical center requirement. Knowledge of coding and documentation requirements to maximize reimbursement. Thorough knowledge of the various forms necessary to initiate any service, test or procedure for a patient. Thorough knowledge and ability to use CPRS to document medical care, etc. Skill in handling crisis situations, effectively calming involved parties and resolving conflicts. Ability to promote good public relations with patient care and associated services for patients. Being able to provide service recovery with patient conflicts and relations with the PACT Team. Thorough working knowledge of computer terminal operation and a thorough understanding of the Vista software utilized in support of patient care and associated services for patients. Ability to learn and teach others to effectively utilize new software programs as they relate to patient care. Thorough knowledge and understanding of VA and medical center regulations, policies and procedures pertaining to eligibility, treatment, medical records and the privacy act (HIPAA). Knowledge of clinical practices and medical care terminology, in order to communicate effectively with staff and patients, and to understand the objectives, organization and procedures of a large clinical service. Knowledge of the Diagnosis Related Groups (DRGs) patient classification system, Current Procedural Terminology (CPT) codes for identification for inpatient procedures.
