Objective/Overview: CUHCC’s mission is to seek health equity in our community by advancing the well-being of diverse people. The clinic is known for providing high quality care to diverse patients and is moving forward in improving timely access to care for patients. CUHCC is building a care model that seeks to improve care at four different points: before the visit, during the visit, after the visit and between visits. This position will focus on providing care after the medical or psychiatry appointment has been completed. This position will work alongside the providers, interpreters, nurses and other members of the patient’s care team. The initial focus of this position will be to serve those who are at high risk and who need immediate follow up upon completion of the visit to ensure that they have a path to the next steps in their care. This position will also follow all UMN and CUHCC policies, procedures, protocols, and manuals as it applies to the duties and assures compliance and other regulatory requirements.
Essential Functions: 1. Referral Coordination: (85%) a) Ensure that referrals particularly for Psychiatry and Therapy are properly scheduled and coordinated with any additional service line such as Community Programs and interpreters; monitors patient attendance to appointments and follow-up with patients; serves as backup for other referral orders as well as other referral functions such as orders for durable medical equipment (DME), Prior Authorizations (medications and services), Home Health Care recertification paperwork and the coordination associated with those services. b) Assist in scheduling follow up appointments and/or referral appointments to other facilities per provider or patient request. c) Coordinate across departments including but not limited to Health Information Management to ensure that all required records, imaging, and/or procedural results are available to the external provider before patient’s appointment. d) Collaborate with other members of the patient care team to ensure patients receive assistance/resources for medications, find transportation, or other community resources (food shelves, groups, shelters) as needed. f) Connects patients to the patient service representatives to apply for public programs and other insurance resources as needed. g) Assist with departed provider panels and transfers of care to new providers when appropriate. h) Ensures appropriate documentation and tracking using the electronic health record. Therapy and Psych Referrals: · Manage internal therapy and psychiatry new referrals (e.g. calling patients on the waitlist, update referral order tracking, notifying referring provider of outcome, documenting outreach attempts, closing referrals) · Manage external therapy and psych new referrals (e.g. same as above) · Oversee scheduling, monitoring schedule templates for each provider in both therapy and psychiatry, anticipating scheduling needs, balancing caseloads, filling schedules, if no show’s rescheduling initial pts to psych and therapy · Managing schedule outreach to new psych and therapy patients · Coordinating with interpreters · Maintaining efficient operations (e.g. connect pts to staff/departments, provide verbal updates between providers about urgent patient needs · Pay attention to details related to psych and therapy flow, reporting trends and system level to supervisor and Mental Health Director · Oversee this and manage BH external specialty referrals such as external Case Management, CTSS, or neuropsychological testing (e.g. calling external providers to schedule appointments, coordinating with health information management to obtain results, communicating scheduled appointments and directions with patient, notifying referring provider of outcome). These referrals can be done by Social Work Triage Specialist interns but will be managed by this role · Coordinating CUHCC group referrals in psychiatry (approximately 4 groups per year) · Communicate updates about status of psychiatry referrals through internal communication channels and external referral sources (e.g. Center for Sexual Health, COPE, Healthcare for the Homeless, Refugee Health Nurses) · Distinguish pt need either therapy or psych (pts aren’t always clear) · Talk to pts about what they can expect at first visit for evals, length, provider · Pt edu about difference btw therapy and psych · Pt needs when scheduling and referring acute needs to appropriate services · Scheduling next appointment only for patients who need additional assistance with this after their visit (versus going to the PSR’s to schedule)
2. Support clinical workflows (10%) a)Supports screening nurse phone calls as needed and serves as a backup for the restricted recipient referrals workflow. b)Work with department managers to draft standard work flows and update procedures as needed c) Other administrative tasks as assigned
3. Other duties as assigned. (5%)
Hourly rate: $19.11 - $23.94
