The Street Outreach Team Member is a mobile-position responsible for conducting outreach activities, providing information and referrals, completing client intakes, performing case management, and direct services to homeless persons living on the streets or places not meant for human habitation.
Provide targeted outreach to the communities of Oak Park and River Forest and other communities as requested according to established safety guidelines to identify homeless persons found living on the streets or other places not meant for human habitation (e.g. storage facilities, forest preserves, etc.).
Offer basic needs and sundries (e.g. food, personal care items, clothing) and provide resources and referrals to other social service venues and agencies including shelter and access to the Support Center.
Use trauma-informed and LGBTQ competent engagement techniques to build a rapport with individuals encountered.
Use evidence-based techniques with clients to achieve consensus on goals, and practice and teach Harm Reduction and the Housing First / Shelter First model.
Provide crisis intervention for individuals in need of immediate assistance related to addiction, domestic violence, or human trafficking.
Work closely with Housing Forward case management and housing team to ensure effective coordination of services.
Work as a team with the other Outreach Worker to alternate responsibilities and negotiate tasks to ensure safety and that all duties are fulfilled.
Communicate and coordinate with program staff from as needed to ensure seamless delivery of service to adults and families.
Provide community outreach and training organizations, businesses and groups regarding the services offered by the Street Outreach Team.
Participate in Housing Forward’s fund raising and public relations events and efforts as needed and to the extent which it does not interfere with other job duties.
Provide age appropriate interaction with homeless youth encountered, connect to youth provider agencies and adhere to all agency guidelines and ethics regarding treatment of youth and services.
Follow the objectives of the program and understand the program’s performance measures.
Collect and maintain program documentation.
Enter data into HMIS daily.
Complete reports (funding, programmatic) reports as assigned.
Be responsible for safety and proper use of cell phones and other equipment.
Respond to calls/requests from the community (municipalities, libraries, schools, etc.) regarding individuals and families found to be living on the streets, cars or other place not meant for human habitation.
Log activities and time as required by the agency and its funders.
Regularly attend team and agency meetings and trainings.
Minimum Qualifications and Skills:
Bachelor Degree required in social services or related field.
Bilingual Spanish and English fluency highly desirable.
Driver’s license, clean driving record, auto insurance, and access to personal vehicle.
Willingness to obtain needed medical screens and immunizations such at TB test.
Computer literacy, including knowledge of computer assessment tools, data base systems and windows environment. Proficient in Excel, Outlook and Word.
Ability to keep appointments punctually, be organized and demonstrate accountability and follow through.
Openness to new ideas and skills and commitment to learning though reading and with hands-on-training.
Ability to express ideas clearly and concisely both orally and in writing.
Ability to problem-solve and make decisions in chaotic and/or stressful situations.
Strong interpersonal and direct communication skills.
Work Environment/Physical Demands:
Constant traveling and driving.
Must be able to lift 25 lbs.
Extreme standing, walking, bending and lifting.
Ability to tolerate being outdoors in all weather for several hours at a time.
Excellent engagement skills, ability to work independently and as part of a team. Knowledge of issues related to homelessness and poverty. Strong interpersonal and problem solving skills.
