The Patient Safety Manager will manage and coordinate the development and implementation of a proactive safety and reliability culture and practice throughout the organization.
The Patient Safety Manager is in charge of the day-to-day operations of the Culture of Safety and Reliability program at the hospital
Responsible for managing the Patient Safety Committee
Works to build and create a positive, open, and transparent culture
Cultivate and improve a culture of trust throughout the organization
Actively promote safe and reliable healthcare through an open, fair and just culture
Uses various methods of culture transformation including HPI (Healthcare Performance Improvement)
Responsible for managing and executing patient safety education and prevention programs including but not limited to Error Prevention training, Safety Rounds, Safety Communications, and safety coach activities
Oversees day-to-day operations of the Daily Check-In meeting, working closely with senior leadership
Collaborates with the Safety Officer to facilitate an environment that prioritizes patient and associate safety and reliability
Manages sensitive patient and associate safety information in a professional manner and strives to always achieve excellence in coworker communication and collaboration
Actively participates in safety related corporate goals and supports partner department in other ways as needed
Provide leadership for safety assessments
Educate other practitioners on the system-based causes for medical errors including root cause analysis, apparent cause analysis, and common cause analysis
Collaborates with risk management in the investigation, review and analysis of safety events.
Consults with management and staff and communicates literature-based ideas regarding effective patient safety strategies to others within the organization
Support the organization in ensuring that the Joint Commission National Patient Safety Goals are met
Work actively with HR on employee safety improvements
Ability to engage and work well with physicians and clinical staff on improvement work around patient safety
Provides data analysis and trending and provides proactive ideas and alerts to relevant areas
Responsible for monthly data reporting on Patient Safety
Develops a mechanism for internal communication of patient safety related information including lessons learned from cause analysis
Is available daily to be visible on the patient floors to address patient safety issues and act as a safety consultant to all staff
Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmental to facilitate that department's/unit's ability to meet its goals and objectives.
Attends and participates in department meetings and participates in multidisciplinary groups as necessary and/or requested.
Performs other related duties as required.
Bachelor's degree required, preferably in a health-related field
Minimum of five years clinical hospital or nursing experience
Knowledge of safety science, high reliability, cause analysis and error prevention in healthcare
Knowledge and experience with medical records documentation
Ability to work independently in a time oriented environment
Project management and program development skills
Experience working in multi-facility health system
