Responsible for oversight responsibility for facility-wide patient safety program. This leadership role will direct others within the facility towards process improvements that support the reduction of patient/health errors and other factors that contribute to unintended adverse patient outcomes. Position provides leadership for safety assessments, coordinates the activities of the patient safety committee, educates other practitioners on the system-based causes for patient errors, consults with management and staff, develops and implements evidence-based programs that support effective patient safety strategies.
Minimum Education Requirements
Bachelor's degree required
Bachelor's degree in Nursing or other clinical discipline preferred
Graduate level education preferred
Minimum Experience Requirements
- Five (5) years of experience in a clinical healthcare environment
- Two (2) years of previous leadership experience, preferred
- Previous experience with regulatory requirements, safety/risk/quality management and education
- Knowledge of Joint Commission hospital standards and CMS regulations and occurrence reporting requirements.
- Experience in clinical operations and/or experience working in performance improvement or quality management areas, preferred
- Experience in managing a patient safety program, including occurrence reporting system, risk management and implementation of patient strategies (eg. proactive risk assessment, FMEA, root cause analysis), preferred
- Proven education and facilitation skills, ability to educate and motivate physician leaders and organizational managers and to effectively facilitate improvement projects, process redesign and risk reduction workshops.
- Ability to create and present quality data using tools such as Microsoft Excel and PowerPoint
Current Patient Safety Certification, preferred
- Develops and coordinates a transparent, data-driven, proactive and preventive facility Patient Safety Program.
- Reviews and refines the program annually.
- Utilizes appropriate forms of recognition to promote patient safety.
- Maintains the knowledge and ability to guide and provide instruction regarding Just Culture and Just Culture algorithms.
- Participates or facilitates meetings, task forces, committees as assigned to promote a "culture of safety".
- Identifies potential areas for continuous quality improvement, patient safety, risk reduction or trends and reports to appropriate committees for resolution.
- Designs and implements educational presentations that facilitate the understanding and implementation of patient safety standards in the organization.
- Promotes the patient safety culture survey with facility staff.
- Serves as a resource for all departments on issues of patient safety.
- Actively leads or participates in patient safety initiatives that promote evidence based safety practices.
- Maintains knowledge of operational trends, appropriate regulatory, licensing bodies and federal/state governmental agencies.
- Investigates patient safety issues using event analysis methodology to conduct clinical incident reviews (Critical Event Analysis, Root Cause Analysis, FMEA-Failure Mode Effects Analysis) of adverse patient events and/or near miss events. Oversees and communicates safety improvementsInterviews staff and physicians to detail occurrence for Patient Safety and Risk reports. Reports occurrences for Peer/Professional Review after interviewing staff and physicians.
- Oversees the management and use of patient error/occurrence information; reviews internal error reports and utilizes information from external reporting programs (eg. ISMP Medication Safety Alert Health Device Alerts).
- Provides comprehensive reports to Hospital leaders, medical staff leaders and community board as appropriate on patient and healthcare errors, near misses and/or dangerous conditions within the facility.
- Prepares reports to comply with reporting requirements for regulatory agencies including State Health Departments, State Medical Device Act and State Board of Nursing. Responds to agency requests.
- Supports and encourages error reporting throughout the organization through a "just culture" occurrence reporting cases to the claims department.
- Collaborates in the development of facility policies/procedures effecting patient safety.
- Recommends and facilitates change(s) to improve patient safety based on identified risks.
- Develops a mechanism for internal communication of patient safety information.
- Designs immediate interventions to prevent reoccurrences of adverse events.
- Mentors managers in patient safety and risk reduction strategies to resolve issues found in occurrences.
- Determines qualifications and competence for all supervised associates to include preparation of performance evaluations and maintenance of attendance and payroll records.
- Provides direction, supervision and support for all supervised associates to maintain their assigned job responsibilities.
- Oversees and coordinates all intradepartmental services for Patient Safety/Risk Management.
- Assists with preparing department budget, including personnel, space allocation and resources necessary to provide designed services.
- Develops and directs other procedural and/or special projects related to the Quality Department.
