Position Summary: We have an exciting opportunity to join our team as a Coord, Bariatric Nutrition.
In this role, the successful candidate Responsible for coordinating Bariatric patient services and providing nutritional support services for morbidly obese patients, including pre and post operative counseling, support groups and continuous monitoring and follow up. Responsible for developing, monitoring and updating processes and related policies and procedures regarding Bariatric patient care. Responsible for developing/acquiring educational materials for morbidly obese patients. In addition, the Bariatric Coordinator/ Nutritionist provides comprehensive individual and/or group nutritional counseling to non-Bariatric patients in the LFHC network, to better manage conditions including diabetes, cardiovascular disease and obesity. The Bariatric Coordinator/Nutritionist will provide all services in accordance with hospital policies, procedures and requirements of regulatory agencies.
Responsible for the successful coordination of patients through the Bariatric Program. Coordinates and collaborates with other personnel to facilitate accurate and efficient flow of Bariatric patients through the system.
Assist in the successful patient compliance through all the disciplines of the Bariatric Program (e.g. Nutrition, Specialty consults, Wellness Program
Assist in increasing the number of patients enrolled in the Bariatric Program
Assist in achieving an average of 3 to 4 surgeries from the Bariatric Program per month.
Conducts pre operative nutritional assessments for morbidly obese patients, and post operative nutritional assessments for patients that have undergone bariatric surgical procedures.
Conducts support groups for morbidly obese patients, and pre and post-operative patients.
Assists in the coordination of, and participation of open houses and support groups. Arranges for guest speakers to appear at open houses and support groups when necessary.
esponsible for researching, planning, developing as necessary, implementing and evaluating education and nutritional materials for Bariatric patients. Also assists in the development of public relation materials for the Bariatric Program as necessary.
Develops, monitors and updates processes and policies and procedures related to the nutritional aspect of morbidly obese and bariatric patients
Acts as a resource for patient care and processes related to the Bariatric surgical program.
Assist in the screening of new patients to determine candidacy for Bariatric surgery. Responsible for fielding phone calls from perspective patients and explaining principles of Bariatric surgery and general questions about the Bariatric program
Participates in meetings with bariatric surgeons and staff as necessary
Copies and distributes educational materials and Bariatric surgical program materials as necessary
Participates in quality management aspects of the Bariatric surgical program as necessary
Assists in the collection and synthesis of patient data.
Demonstrates knowledge and skills necessary to provide care based on physical, psychosocial, educational, safety and age-related criteria pertinent to the patients served in his/her assigned area.
Performs Comprehensive Nutritional Assessment for patients according to FHC standards and guidelines
Reviews patients medical records to obtain and interpret data and information relevant to disease management
Develops and documents appropriate Nutritional Care Plan.
Documents on the multidisciplinary teaching record diet instructions provided to patients and/or families, and the response of the patient to the prescribed diet, including the ability to verbalize the rationale of the diet.
Conducts nutrition education classes for groups of patients, family members or the community.
Confers and coordinates nutrition care with other members on the Healthcare team (Physician; nurse; pharmacist; social services; etc.)
Evaluates and documents the effectiveness and outcomes of nutrition therapy during patients scheduled follow up visits
Maintains current nutrition knowledge through participation in continuing professional education meetings/seminars
Participates on committees and multidisciplinary teams and conferences assigned
Provides nutrition consults per request of patient and multidisciplinary team.
Reviews, updates and ensures adequacy of nutrition educational materials to prevent shortage or disruptions in patient care, or the performance of duties
Maintains established departmental policies and procedures, objectives, Quality Improvement Programs, Safety, Environmental and Infection Control
Appropriately manages time to complete goals
Demonstrates a sense of teamwork.
Minimum Qualifications: To qualify you must have a Bachelor of Science in Food and Nutrition Sciences, completion of nutrition AP4 or internship through an accredited college, Master of Science in Health or Nutrition preferred. Two to three years minimum experience in a clinical or outpatient environment practicing nutritional evaluations and counseling. Required License, Registration, Certification: Dietetic registration through the Commission on Dietetic Registration, licensure through the State Education department, active membership in the Academy of Nutrition and Dietetics. Certified Diabetes Educator preferred.
